Two Workers Killed in Chemical Plant Explosion

Two workers at AL Solutions, Incorporated, a titanium and zirconium processing facility in New Cumberland, West Virginia, were killed in an apparent plant explosion on December 9th; a third worker was severely burned. A four-person investigation team from the U.S. Chemical Safety Board (CSB), led by Investigator-in-charge Robert Hall, was deployed to the scene. The cause of the explosion is still under investigation.
The CSB is an independent Federal agency whose role is to investigate industrial and chemical accidents. CSB investigates these incidents by investigating the processes, equipment, associated standards, and safety management systems in place at industrial facilities. CSB Board members are appointed by the President and confirmed by the Senate.
